• Referring community cardiologists can call our Betty Irene Moore Children鈥檚 Heart Center at (650) 721-2121聽and request a referring provider form for a patient evaluation.
• Stanford-based cardiologists can enter an ambulatory referral for our program to the cardiology team, noting 鈥淢D evaluation for fitness program,鈥� and email heartfitness@stanfordchildrens.org so our team is aware of the referral.
