CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

Getting Here
From CA-24 East
Head east on CA-24 East. Keep left at the fork; follow signs for I-680 North/Sacramento/Concord and merge onto I-680 North. Take the Treat Boulevard exit toward Geary Road. Keep right at the fork to continue toward Treat Boulevard. Keep right at the fork; follow signs for Silicon Valley College and merge onto Treat Boulevard. Turn right onto Bancroft Road. Turn right onto Ygnacio Valley Road. 1776 Ygnacio Valley Road will be on your right.
From I-680 North
Head northwest on I-680 North. Take the Ygnacio Valley Road exit. Turn right onto Ygnacio Valley Road. Make a U-turn at San Carlos Drive. 1776 Ygnacio Valley Road will be on your right.
From I-680 South
Take the exit toward Treat Boulevard/Geary Road. Keep right at the fork to continue toward North Main Street. Keep left at the fork and follow signs for Treat Boulevard. Turn left onto North Main Street. Turn left onto Geary Road. Continue onto Treat Boulevard. Turn right onto Bancroft Road. Turn right onto Ygnacio Valley Road. 1776 Ygnacio Valley Road will be on your right.
