CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

Getting Here
From Highway 101 South
Take the CA-85 South exit toward Santa Cruz/Cupertino. Continue onto CA-85 South. Take the Camden Avenue exit. Turn left onto Camden Avenue. 6475 Camden Avenue will be on your right.
From Highway 101 North
Keep right at the fork to continue on CA-85 North; follow signs for Cupertino/Mountain View. Take the Almaden Expressway exit. Turn left onto Almaden Expressway. Turn right onto Camden Avenue. Make a U-turn at Trinidad Drive. 6475 Camden Avenue will be on your right.
From Highway 280 South
Take the exit onto CA-85 South toward Gilroy. Take the Camden Avenue exit. Turn left onto Camden Avenue. 6475 Camden Avenue will be on your right.
From Highway 880 South
Continue onto CA-17 South. Take the exit toward Gilroy. Merge onto CA-85 South. Take the Camden Avenue exit. Turn left onto Camden Avenue. 6475 Camden Avenue will be on your right.
From Highway 680 South
Continue onto I-280 North. Take the CA-87 exit toward Guadalupe Parkway. Keep left at the fork; follow signs for CA-87 South/Guadalupe Parkway. Keep left; follow signs for CA-87 South and merge onto CA-87 South. Take the Almaden Expressway exit. Merge onto Almaden Expressway. Turn right onto Camden Avenue. Make a U-turn at Trinidad Drive. 6475 Camden Avenue will be on your right.

Saturday mornings:
We open our phone lines at 8:30 a.m. to schedule appointments for urgent sick visits only. Patients from both locations are seen at the Samaritan office on Saturday mornings. There is an extra fee billed for these urgent visits.
Sundays, major holidays & after hours:
Our offices are closed. For urgent issues only, call either of our office telephone numbers for instructions. One of our physicians is always on call for emergencies. He or she is in touch with our answering service as needed.
