CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

If you are unable to complete CHADIS before your visit, please arrive 15 minutes before your appointment time in order to complete the health questionnaires in our office.

Exit onto Foothill Road. Turn left onto Laurel Creek Way. 5720 Stoneridge Mall Road will be on your left.
From I-580 West
Take the San Ramon Road exit toward Foothill Road/Dublin. Turn left onto San Ramon Road. Continue onto Foothill Road. Turn left onto Laurel Creek Way. 5720 Stoneridge Mall Road will be on your left.
From I-680 North
Take the Stoneridge Drive exit. Turn left onto Stoneridge Drive. Turn right onto Foothill Road. Take the first right onto Laurel Creek Way. 5720 Stoneridge Mall Road will be on your left.
From I-680 South
Take the Stoneridge Drive exit. Turn right onto Stoneridge Drive. Turn right onto Foothill Road. Take the first right onto Laurel Creek Way. 5720 Stoneridge Mall Road will be on your left.
