CHADIS Login
CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

From I-880 South
Take exit 17 for Mowry Avenue toward Central Fremont. Use the left two lanes to turn left onto Mowry Avenue. Make a U-turn at Civic Center Drive. 2299 Mowry Avenue will be on your right.
From I-880 North
Take the Mowry Avenue exit toward Central Fremont. Use the right two lanes to turn right onto Mowry Avenue. Make a U-turn at Civic Center Drive. 2299 Mowry Avenue will be on your right.
From I-680 South
Take exit 16 for Mission Boulevard/California 238. Turn right onto Mission Boulevard. Use the left two lanes to turn left onto Stevenson Boulevard. Turn right onto Civic Center Drive. Turn left onto Mowry Avenue. 2299 Mowry Avenue will be on your right.
From I-680 North
Take exit 15 for Washington Boulevard toward Irvington District. Keep right at the fork, follow signs for Irvington Drive and merge onto Washington Boulevard. Slight right onto Fremont Boulevard. Turn right onto Stevenson Boulevard. Use the left two lanes to turn left onto Civic Center Drive. Turn left onto Mowry Avenue. 2299 Mowry Avenue will be on your right.
